The program is designed to meet the needs of facilities and individuals who have had a recent hospital or skilled nursing stay and are now returning home. This program’s goal is to provide an extra layer of support and care for people who are living with chronic conditions and are at risk for re-hospitalization.
Typically, skilled nursing centers may discharge up to 15 percent of their patients home each month. Patients often experience disruption in their pharmacy care upon returning home, confusion about their new medication regimen, and the physical challenge of traveling to a pharmacy to access their medications. With in-home pharmacy care in place, the patient avoids this disruption, reduces their adverse drug events, and ultimately reduces unnecessary hospital readmissions or emergency room visits.
TransitionCare provides easy, accessible, and affordable pharmacy care directly to the home setting and is a seamless experience for patients and their existing providers. This program assists facility partners to provide a better continuum of care and reduces the medication management burden on discharge planning teams. Early studies indicate that Centennial's support reduces readmissions to under 9 percent, increases adherence measures for diabetic and cholesterol medications to over 98 percent, and has a 97 percent patient retention, with high patient, provider, and facility satisfaction.

TransitionCare is currently available to discharging facilities and hospitals in South-Eastern Pennsylvania, Southern New Jersey, and New Castle County Delaware.
